Boise Physical Medicine & Rehabilitation (PM&R):

Please contact the site coordinator Melissa Honsinger at Boise PM&R at least TWO weeks before the start of the rotation.

You also will spend time at the St. Luke's and St. Alphonsus rehab units during your rotation. Each has a set of pre-rotation requirements that must be completed before you begin.

St. Luke's: Cynthia Probasco will contact you regarding your paperwork and pre-rotation training. These must be complete no less than THREE weeks prior to the beginning of the rotation.

St. Alphonsus: The pre-rotation paperwork is due no later than 30 days prior to the clerkship start date. Late or incomplete paperwork will not be accepted and you would need to reschedule your clerkship. They will not send you reminders, so please complete the credentialing items ASAP.

Students should visit https://www.saintalphonsus.org/graduate-medical-education, complete ALL required items, and and return them directly to Saint Alphonsus via e-mail or fax. Contact information provided at link above.

  • Clerkship/Rotation Form (UWSOM students not required to obtain Supervising Physician’s Signature – Saint Al’s will collect signature)
  • Confidentiality Agreement
  • Student Manual – Read and complete Student Orientation Worksheet
  • Provide Influenza vaccination verification if rotating between Nov 1 – March 31

Letter of Good Standing and Certificate of Liability are provided by UWSOM on your behalf directly to site.

The Idaho State Board of Medicine requires all clinical learners to be registered in the state prior to completion of any clinical rotations taking place in Idaho.
The Idaho WWAMI clinical office in Boise will assist all students in completing this requirement at the beginning of the academic year and as students add clerkships through the year. If you have already registered this academic year, no action required from you at this time. Otherwise, note that the forms MUST be returned to their office by the due date requested. You will not be allowed to begin your rotation until you are registered by the Idaho State Board of Medicine.
