Chronic Care Clerkship Forms

Mid Rotation Feedback

All students must meet with his/her clerkship preceptor at the end of the second week of the rotation to discuss core experiences listed on the Mid-Rotation Feedback Form. This conversation will give the student and the preceptor an opportunity to informally discuss the students' performance so far and to determine which core experiences still need to be completed before the end of the rotation.

After the conversation, students must fax the completed form to the clerkship coordiantor at (206) 616-3908. Completed forms must be received before the end of the third week of the rotation. Completing the mid-rotation feedback conversation, and this form, is a requirement for passage of the clerkship.

Patient Case Presentation

All students will conduct a brief 10-12 minute presentation, followed by a 15 minute discussion, of their comprehensive patient case write-up to a small student group and faculty member on the last day of the rotation. The student's presentation is evaluated by the faculty member on the form below or via Catalyst.
